Yahoo should stop awarding Holds to pitchers given a Win by Scorer. A hold is like a mid-game Save, can’t get both.

As a statistic, holds are designed to credit late-relief pitchers who are not closers. ... A pitcher cannot receive a win or a save in a game in which he records a hold. However, more than one relief pitcher can record a hold in a single game.

The specific scenario is when the starter goes less than 5 innings, so he can't get the win, but he leaves the game in a save situation (1-3 run lead, or 4-5 run lead with the tying run batting or on deck). The reliever who comes in next (and maybe others after him), if he holds the lead, would be granted a hold as soon as he's removed from the game. At the end of the game, the official scorer, will assign the Win to the pitcher he feels was most effective pitching in the game. Many times, this is the guy who came in right after the starter if he of course holds the lead. Thus, he'll get both a win and a hold.
It happens rarely, but I'd be surprised if it's a dozen times per season.
It is possible for a P to get an H and a W. It happens about a dozen times per season.
