Fix the 'you have to create a new password every time you log in'

I have to create a new password every time I login. Any fixes?

    I have seen these posts for at least 2 years, still no fix. It used to be every month or so, but now it is every single time I log in. One fix was to use Firefox but that doesn't work, neither does emptying the cache
