Instead of flip-flopping their status between Out and Questionable every week, why not leave their status as Out until they are actually cleared to return? Benefit:
This would allow owners of these players to leave them in their IR slot until they are active, which in turn allows them to keep their replacement player, instead of dropping the replacement into the FA pool at the beginning of the week, only to have to pick someone up later for the same purpose.

*Note: Players who are newly-designated as "Questionable" after having played the previous week would not be eligible for IR until they actually are ruled out.
